Immunization Compliance

All freshmen at USF must sign a mandatory Medical History Immunization Form, and be able to provide proof that they have received two MMR shots or the measles (Rubeola) and German Measles (rubella) vaccination since their first birthday.

Furthermore, the University of South Florida requires that you must provide proof of vaccination against meningitis and hepatitis B or provide a waiver document duly signed for both vaccines.
